Case Management with Multi-Problem Families: Achieving Better Outcomes for Clients
Course Overview
This two day workshop covers case management and case planning for families that present to an agency with a single problem, but on engagement multiple issues and problems are identified. Participants will explore the basic principles of case management when engaging and working with multi-problem families and the development of appropriate plans.
Participants will develop or enhance their skills in:
• Assessment and case management • Identifying, in conjunction with the family, the specific problems to be addressed • Developing a viable intervention plan to address identified problems • Establishing goals, outcome measures and time frames • Developing a model for assessing whether the family has made changes to address identified problems
• Working collaboratively with other agencies
Who is this workshop suitable for? This workshop is relevant for all human service professionals who work with clients experiencing multiple problems and issues.
By the end of this workshop participants will be able to:
- Appreciate the basic principles of case assessment and the complexity of case management when working with families
- Undertake initial assessments which focus both on the presenting problem and underlying factors
- Identify and develop strategies to engage families and relevant agencies in collaborative case management
- Articulate a case management plan and assess the effectiveness of the case management plan in addressing needs
- Engage relevant agencies to own and prioritise their involvement in the case management process
